Our receptionist will contact you prior to your appointment with a reminder. If you don’t hear from us by the afternoon of the day before your appointment is due, please contact us to confirm the appointment.

A questionnaire will be sent to new patients to Retina & Vitreous Centre. Filling out the form before your visit will help us to look after you better, and the visit will be less rushed.

Please remember to bring the referral letter and prepare a list of your medications, including any eye drops. If you wear glasses, please bring them also.
